**Description:**
**Summary:** Provides assessment and crisis intervention; plans, organizes, and implements social work services for patients and families. Provides long term care planning for patients/families. Responsible for complex discharge planning. Intervenes as needed with hospital patients with mental health problems or substance abuse problems. Provides assessment with case management for pediatric patients.
**Essential Functions:**
1. Acts as liaison with nursing/ancillary/medical staff and community agencies to facilitate problem solving and coordinate services. Acts as an educational resource. Acts as consultant to Employee Health Program including but not limited to policy development and post-exposure follow-up.
2. Interviews patients and family members to obtain information about home environment, family relationships, health history, and patient's personality traits. Communicates family issues to other staff members. Participates in planning conferences.
3. Locates, coordinates, monitors, and assists in determining eligibility of social, medical, financial, and other services to meet the client's needs.
4. Intervenes in emotional crisis situations as they involve patient, families of patients, or nursing personnel. Effectively identifies predictable problems and plans and provides patient care, based on the age of the patient served.
5. Collaborates with primary nurse/physician in discharge planning, which may include referral for psychiatric follow up. Aggressively seeks appropriate placements to other settings/level of care as soon as medically stable for discharge, assuring a safe disposition.
6. Gathers and documents data necessary to assess emergency situations and make referral to the appropriate level of care.
7. Provides ongoing, supportive counseling with patients/families/staff, assisting them with crisis situation, and meeting social and emotional needs relevant to advanced illness, death, and bereavement.
8. Assists patients and families to understand, accept, and follow medical recommendations. Utilizes resources to help patient resume life in the community or to learn to live with the disability.
9. Assesses abuse, exploitation, and neglect of children and adults. Reports in a timely fashion to the appropriate agency.
10. Prepares and maintains required documentation on each case. Demonstrates proficiency at hospital wide and/or department specific computer software and technology.
**Qualifications**
**Education:** Social Work (BSW or MSW) or related degree such as Counseling or Psychology.
**Experience:** Two (2) years clinical experience. If degree is in related field such as Counseling or Psychology, two (2) years applicable clinical experience working in a hospital or larger health care organization is required.
**Skills:** Handles emergency or crisis situations. Requires judgment/action which could result in death of patient.
